Making an Appointment (Outpatients)
Your primary care physician should refer your child to our clinics and likely will schedule the appointment for your child to be seen at a clinic that is most convenient to you.
When you come for your appointment, please bring the patient's insurance information. If coming to the clinic in Chapel Hill, please also bring the patient's UNC medical record card (if the patient does not have a UNC card, it can be obtained at the time of the appointment). Prior to your appointment, you should make sure that your referring physician has forwarded all medical records to the clinic where the patient will be seen.
