Referrals

Referring physicians can refer a patient for all of our clinics by calling 984-974-1401. Once the referral is made, please complete the Referral Form and fax it with all pertinent medical records to 919-966-2423.

After the physician referral is made, parents should call 984-974-1401 to schedule an appointment time convenient for their child.
